Awards Support Letter Form

The following form may be used to provide the following pieces of the nomination packet for both the Distinguished Service Award and the Gatling Award:

A minimum of three (3) and a maximum of five (5) letters of support. For the Gatling Award, one must be from the nominee's supervisor .
A vitae or resume of the nominee (optional)

Choose the award you wish to nominate for:	Michael C. Jackson Distinguished Service Award Deadline March 31 Read award criteria before nominating!
	Herman and Emma Gatling Award for Outstanding Science Education Leadership Must be a member, Deadline October 15 Read award criteria before nominating!
